Premiering in Chevy Chase, MD/NW D.C. and Falls Church, VA

Many elders need better access to primary care due to advanced illness or disability that make it hard to get to the doctor’s office.  If you or a family member face this challenge, Capital Caring Health can now help. 

Beginning December 2019, in selected zip codes, our new Primary Care at Home (PCH) program will help elders maintain their dignity and independence by bringing full medical and social services to the home.  The program will offer primary and urgent care at home, coordinate specialty and hospital care, and arrange daily support services as needed. 

A dedicated team of doctors, nurse practitioners, social workers and support staff provide diagnosis, treatment, daily support and state-of-the-art mobile technology.  We will be available 24/7 by phone and can make urgent visits within 24 hours. This approach lessens the need for 911 calls, ER visits, and hospital stays. The main goals are to offer expert care and peace of mind for elders and families to support aging in place.

Our first two PCH teams will serve areas in Chevy Chase, MD/NW D.C. and Falls Church, VA.  We will add more neighborhoods as the program grows. We serve people who are 65 and over, and have serious chronic illness and physical or cognitive disability. 

Primary Care at Home is part of Capital Caring Health’s Elder Health services which help older adults with advanced illness.  The effort is led by our new Chief of Geriatrics, Dr. Eric De Jonge, who has over 20 years of experience in leading programs for elders and their families.
