HomeCare.com’s caregiver technology will allow Capital Caring Health to achieve scale and optimize the outcomes of each family served.
Falls Church and McLean VA – Dec. 4 — Capital Caring Health, one of the leading non-profit advanced home care and hospice groups in the nation has entered into a partnership with HomeCare.com—the preeminent digital exchange for healthcare professionals in the U.S. HomeCare.com maintains an online platform that connects more than 300,000 credentialed caregivers with families who need care at home. All participating caregivers are licensed, background checked nursing professionals.
A Unique Approach to Caregiving: The HomeCare.com App. will match caregivers directly with families under the care of Capital Caring Health. This match will be based upon the families’ specific needs, location, and schedule. The app tracks activities of daily living and notes all changes in conditions to assist the Capital Caring Health medical team in knowing what is happening at all times in the patient’s home. With this approach both groups can measure quality metrics and drive better outcomes for each patient.
Families Can Select from a Suite of Services: Through HomeCare.com families can interview caregivers who have indicated they are available so that both parties start off in synch with each other. Additionally, families can access the Capital Caring Services Hub which organizes meal delivery, transportation, pet care, and telehealth services, providing an unmatched level of convenience and service to the patient and their family.
Mobile App Peace-of-Mind: The HomeCare.com mobile app shows the family what services are provided in the home including GPS verified check-ins and daily visit reports. This feature allows family members to know their loved one is being cared for properly even if the family members are elsewhere during the day or living in other locations not close to their loved one’s home.
Recent studies show the Social Determinants of health like diet management and caregiver companionship are just as important to outcomes as the medical side of patient treatment. Having in -home activity visibility has been a missing caregiving element for many years but now with HomeCare.com technology HomeCare.com and Capital Caring Health can track the data needed to measure and improve patient outcomes.
