Who We Serve

Elders (age 65+) who are home-limited and have one or more chronic conditions.

What We Do

  • House Calls: By doctors, nurse practitioners, and social workers.
  • Mobile Technology: We offer in-home diagnostic tests (blood work, X-rays, ultrasound) and an electronic portal for communication with our team.
  • 24/7 Coordination of Care: Our team is available 24/7 by phone. We arrange primary and urgent care, specialists, hospital and support services needed to age at home with dignity.
  • Family Time: We work closely with families so all understand the plan of care.

24/7 Access

Seniors enrolled in the Primary Care at Home program may keep their existing specialists while receiving primary and urgent care from us in their place of residence. Our care team, consisting of physicians, nurse practitioners (NP) and social workers, offer 24/7 access to address urgent concerns, providing peace of mind for patients and family.

Added Benefits

And now, Medicare beneficiaries enrolling in PCH may also be eligible for a special enhanced care program that could save them money, provide additional high-touch services, and offer flexibilities in the types of services they have covered.
