Capital Caring Health launches Primary Care at Home, delivering medical care to older patients in the comfort of their homes
At the forefront of emerging models of at-home delivery of needed services, older home-limited adults in the Washington, D.C. region now have access to full medical and social services delivered in the comfort of their own homes. The service is provided by Capital Caring Health’s new Primary Care At Home (PCH) program.
This comprehensive program follows at-home service trends in many sectors across the nation, making it easier for people to shop, eat, bank, and catch a ride by introducing at-your-doorstep services.
The Capital Caring Health Primary Care at Home program provides people with a dedicated care team equipped with the latest mobile health technology to diagnose and treat illnesses, make urgent care visits, coordinate specialty and hospital care, and help manage daily routines. Capital Caring Health created PCH to reduce the need for costly, inconvenient ER visits, 911 calls, and hospital admissions among the area’s senior population.
This ground-breaking program is led by Eric De Jonge, MD, chief of geriatrics at Capital Caring Health and a pioneer in the field of home care.
“Limited mobility makes it harder for some people to get the care they need to manage existing illnesses and prevent new ones,” said Dr. De Jonge. “By providing primary care at home, we fulfill all the care needs, ensuring that mobility issues or lack of transportation don’t stand in the way of feeling better.”
To be eligible, area residents must be least 65 years old and have a chronic illness and physical or cognitive disability. Residents who qualify must change their PCP to our team, however they can keep their specialists.
“We want to make it easier for people to get the care they need, when and where they need it, avoiding lapses that could result in more serious illnesses,” said Dr. De Jonge. “Providing primary care makes it easier for people to get the primary care they need now.”
The program is currently offered to residents of Chevy Chase, parts of Northwest, D.C.; and Falls Church. Capital Caring Health intends to expand the program to other parts of the region in the coming months.
